Mini Movers Registration

I understand that participation in dance activities involves certain risks and hazards, including but not limited to physical injury. I acknowledge that these risks cannot be eliminated entirely, and I voluntarily assume all such risks on behalf of my child and caregiver(s) present.

In the event of any injury or medical emergency, I authorize the staff and/or volunteers of Canal Street Church and/or Porch Swing Soul Care to seek and obtain medical treatment for my child or caregiver(s), including emergency medical services if deemed necessary. I also agree to be responsible for any medical expenses incurred as a result of such treatment.

I hereby release, waive, discharge, and covenant not to sue Canal Street Church and Porch Swing Soul Care, its instructors, employees, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child/caregiver(s) while participating in the dance class.

I grant Canal Street Church and Porch Swing Soul Care the right to take photographs and/or videos of my child/caregiver(s) during the dance class for promotional and marketing purposes. I understand that these materials may be used in print, online, and in other forms of media.  If I have concerns about my image being used I will speak with the instructor prior to attending class.

I agree to ensure that my child/caregiver(s) arrives on time for each class and is appropriately dressed and prepared. I will communicate any relevant medical conditions or concerns to the dance instructor. Myself or another qualified and authorized adult caregiver will participate in the class alongside my child.

I understand that I as the caregiver am responsible for managing my child and his/her behavior during the class.  We commit to being respectful and cooperative to all participants.  I acknowledge that I will attend to my toddler's needs, such as taking a break or helping them return to a calm state if they become overstimulated or overwhelmed.

I acknowledge that regular attendance is important for my child's progress in the class.  Since available spots are limited, we will commit to attending every class.  If I am not able to attend, I will notify the instructor as soon as possible. I understand that, because this class receives voluntary donations, there will be no refunds given.

I agree to maintain open communication with the instructor regarding any questions, concerns, or feedback related to my child's/caregiver’s participation in the dance class, or logistics.  Hello@porchswingsoulcare.com is the primary means of contact for the instructor.

I have read this waiver and fully understand its contents. By signing up for the class, I indicate my voluntary acceptance of all terms and conditions stated herein.
